The Medical Marijuana Registry (MMR) is sharing these tips from the Marijuana Enforcement Division (MED) to help protect your personal information when shopping for medical marijuana.
Protect your personal information before shopping for medical marijuana
You can hide (redact) the following information by covering it completely with a black marker.
Social security number.
Date of birth.
Specific medical diagnosis or condition fields.
The reason for needing more plants.
The reason for any changes to your provider certification form (Amend Reason).

Important reminders
Use a black marker to cover the information completely.
Make sure all other information listed on the provider certification form remains visible so the dispensary staff can verify your identity and your plant and oz count for the purchase.
Keep a copy of your original provider certification for your records.
